New Cadet Registration Package (Forms & Documentation):

To be eligible for membership as a Cadet one must:

  1. be a legal resident of Canada:
    NOTE: A legal resident of Canada is a Canadian citizen, a landed immigrant, or, the dependant of a person who is lawfully resident in Canada on a temporary basis for the purpose of education or employment.
  2. be at least 12 years of age and not have attained 19 years of age;
  3. provide proof of provincial health insurance coverage or equivalent;
  4. not belong to another corps or squadron.


  1. Application for Membership in the Canadian Sea, Army or Air Cadets, CF 1158 *Note: Step 1 now kicks off online at 
  2. Cadet Code of Conduct
  3. CATO 12-50 Annex A – Inspection Consent Form
  4. Sponsoring Committee Cadet Registration Form
  5. New Cadet/Recruit Uniform Measurement (opens in Google Forms)


  1. Proof of Identity (a valid Canadian passport;  a Canadian birth certificate; a valid resident card; OR a valid identification card issued by a Canadian provincial or federal government agency) – original must be presented
  2. Proof of Health Insurance (e.g. BC Services Card) – original must be presented
  3. A copy of the court or tribunal decree or judgement. (If in form CF 1158 under Section 5, you check “Yes”)
  4. A copy of the relevant pages from the court decree or judgement or separation agreement. (If in form CF 1158 under Section 6, under “Access Rights”, you check “Limited” for either Parent/Guardian)


**Additional Medical & Food Service Forms may be required based on items indicated on the Application for Membership CF 1158  form.

  • Detailed Health Questionnaire (Required if, in Section 7, you answer “Yes” to Q1, 3, 4, 5 or 6)
    • Will be provided by the Administration Officer based on entered membership data
  • Anaphylaxis Forms Package (as determined by our Personnel system based on additional medical details)
    • Anaphylaxis Consent Form for the Canadian Cadet Organization
    • Anaphylaxis Emergency Plan
    • Participation By A Cadet With Food Sensitivity
    • Participation By A Cadet With Non-Food Related Anaphylaxis

Returning Cadets:

  1. Annual Validation Form
    • Please return page 2 (Annual Validation) only. Do not return the Detailed Health Questionnaire by e-mail.

      IF you have a Detailed Health Questionnaire: there is a deadline to submit pages 5-8 to Victoria. Otherwise, your ability to participate in Cadet activities may be limited.
